The Weekend Triage Nurse role is responsible for the coordination of after hours phone calls for all home health patients. The triage nurse is responsible for the calls regarding patient care and referrals after hours. |
Duties: |
| The LPN will follow all department procedures as it relates to accepting referrals, and patient care issues. This LPN will work with a high degree of independence to assist patients with clinical and operational concerns/questions, providing sound clinical resolutions and extraordinary customer service. LPN will perform clinical telephone triage follow-up with clinical referral sources when necessary. |
Qualifications: |
A valid current LPN License from the state in which he/she would be practicing is required. Ability to manage stressful situations, assess patient complaints and provide clinical education over the phone. Candidates must have a minimum of two years medical/surgical experience within the past 5 years or 1 year HH Triage experience. At least 1-year home care experience is preferred. Excellent verbal and written communication skills as well as strong organizational, time management and problem solving skills, flexibility and the ability to multi-task are required. This position will be triaging for the Triad during the weekend. |
