The Transition to Home Specialist is responsible for ensuring continuity between the hospital and the home setting in order to maximize patient satisfaction of home health services; to optimize patient safety, comfort, and well-being; to improve awareness and confidence among healthcare professionals, physicians and patients regarding current home health options and capabilities; to coordinate referrals to ensure timely admission and appropriate patient care based on doctor's referrals. The Transition to Home Specialist will also promote our products and services to doctor's offices, hospitals, and clinical practices to grow our business. |
Duties: |
Must be very knowledgeable of all our products and services. Will cold call and make in person visits. Must be able to build relationships and have great follow-up. |
Qualifications: |
2 year college degree required. 4 years of clinical exp, customer service, retail sales, or health care sales is required. Previous work experience in the Health Care Sales field is a plus. Clinicians are preferred. |
