The mission of Arbor Hospice is to give comfort, assurance and care to families and patients who have life limiting illnesses, and to educate and nurture others in this care
The role of the Nurse Liaison is to develop and execute marketing programs designed to drive brand awareness, educate key constituents, recruit resources, increase referrals/admissions/ conversion rate and support development efforts within the hospital setting. Develop and sustain business partnerships to generate early and appropriate referrals. Meets and/or exceeds budgeted referral volume goals. Identify new opportunities for partnerships. Establish working relationships and open lines of communication between appropriate personnel within Arbor Hospice, patients and families, physicians, discharge planners, nursing and facility staff.
The registered nurse plans, organizes, directs and provides hospice care while in the hospital. The professional nurse builds from the resources of both the hospital and community to plan and direct services to meet the needs of individual and families for discharge planning.
This position will be working with 2 local health systems, evening and weekend coverage. |
Duties: |
1. Develop and sustain strong relationships with referral sources with a focus on increasing referrals and admissions to Arbor hospice. Primary focus on hospital accounts. Interacts with all health care team members, clients and their families to answer questions regarding hospice services and sign on to service.
2. Provide exceptional, world class customer service
3. Populate a central/shared marketing database with key hospital contact and activity information
4. Develop, implement and take responsibility for an annual hospital plan to increase referrals and conversion rate
5. Assess needs for and conduct educational in-services for providers
6. Timely completion of marketing and clinical documentation
7. Make recommendations on marketing and clinical opportunities.
8. Manage and coordinate the admission process for each referral. Document as required and provide updates to the Access Center. Identify and resolve any process issues for referral sources.
9. Discuss options and alternatives with physicians regarding hospice services. Verify and communicate initial assessment of hospice needs and plan for services. Routinely follows the progress of prospective patients.
10. Submit information on competitive activity/ trends within the hospital.
11. Interface with Provider Relations Representatives, Community Outreach Representatives and clinical team within the same geography to ensure appropriate coverage
12. Work with the Director of Sales and the Clinical Manager on targeting and growing referrals and admissions
Patient Care
1. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es).
2. Provides professional nursing care by utilizing all elements of nursing process.
3. Assesses and evaluates patient’s status by:
A. Writing and initiating GIP plan of care
B. Regularly re-evaluating patient and family/caregiver needs
C. Participating in revising the plan of care as necessary
4. Initiates the plan of care in accordance with current professional standards and practice, and makes necessary revisions as patient status and needs change.
5. Uses health assessment data to determine nursing diagnosis.
6. Develops a care plan that establishes patient and family centered goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
7. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
8. Counsels the patient and family in meeting nursing and related needs.
9. Provides health care instructions to the patient/ caregiver as appropriate per assessment and plan.
10. Acts as Case Manager and assumes responsibility to coordinate patient care for assigned caseload.
Communication
1. Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate.
2. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
3. Communicates with hospital health related persons to coordinate the care plan.
4. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group and hospital staff.
5. Provides and maintains a safe environment for the patient.
6. Assists the patient and family/caregiver and other team members in providing continuity of care with discharge planning.
7. Works in cooperation with the family/caregiver and hospice interdisciplinary team to meet the emotional needs of the patient and family/caregiver.
8. Attends interdisciplinary team meeting as indicated by clinical manager.
Additional Duties
1.Participates in on-call duties as defined by the on-call policy.
2. Ensures that arrangements for equipment and other necessary items and services
are available in a timely manner.
3. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
4. Fulfills the obligation of requested and/or accepted case assignments.
5. Actively participates in quality assessment performance improvement teams and activities as well as other sponsored in-service training.

Qualifications: |
1. At least 2 years Hospice experience and /or 3 years sales experience with a positive results in a healthcare environment . One (1) to two (2) years of recent acute care experience in an institutional setting desirable.
2. RN license in the State of Michigan and maintains such licensure in accordance with applicable laws and regulations and perform within the scope specified; CPR certification. Successfully obtain CHPN certification within 6 months.
3. Prolonged or considerable walking or standing. Able to lift up to 50 pounds in order to lift, position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping bending, kneeling and/or crouching. Visual acuity and hearing to perform required nursing skills.
