A registered nurse is always on call, 24 hours, to answer questions and to provide assistance and support when needed. We will provide care to every eligible patient in our service area, regardless of medical conditions, lifestyle, religious, cultural or ethnic background. We believe all patients who are eligible for hospice should receive hospice. After all this is the time When Life Matters Most....

Duties:

Registered Nurse Case Manager:

• You will act as the RN Case Manager for Hospice Patients.

• The Hospice RN Case Manager is responsible for the planning, coordination, provision and documentation of care for terminally ill patients from day of admission through day of discharge.

• Works in compliance with federal and state regulations, reimbursement guidelines as well as the organization's policies and procedures.

• Assures quality care by reviewing and collaborating with the multidisciplinary team including RN, Social Worker, C.N.A., Chaplain and Volunteers in conjunction with the Patient Care Coordinator, RN.

• The Hospice Nurse participates in quality control programs.

• The position reports to the Patient Care Coordinator, RN.

• Works with IDT to develop and implement hospice plan of care.

• Supervises Hospice Aides and LPNs.

Job Specifications:

• Registered Nurse with a current nursing license.

• Knowledge of or willingness to learn hospice care.

• Minimum one year nursing practice or equivalent experience required.

• Hospice, Oncology, or Home Health and/or long-term care preferred.
